Submitting Referrals
Referrals can be submitted to our secure referral inbox at referral@usevantagepoint.com
Please include the following documentation when submitting referrals:
- Individualized Plan for Employment (DR 215)
- Consent to Release and Obtain Information (DR 260)
- Initial Intake Case Notes
- Vantage Point Referral Form (which can be downloaded at the bottom of this page)
- Any other relevant documentation or resumes are greatly appreciated
What to Expect after Referral Submission
Once a referral is received, our team reviews the documentation and will assign a Job Developer based on client needs and staff capacity within 30 days.
If a client needs priority assignment due to risk of homelessness or other extenuating circumstances, please reach out to the Program Manager, Bianca Allende Boyd, at bianca@usevantagepoint.com
After a referral is assigned, the Job Developer will contact the client directly to schedule an intake meeting. Counselors will receive an introductory email and intake report soon thereafter.
Our team communicates regularly with DOR Counselors regarding client progress, milestones, and case updates to ensure shared success.
